The ? you find in the #11 is probably Isaac finding that he can kill himself in the chest and his death wish starts to appear, making the Epilogue come first, then Cathedral, then #11, then Sheol or Chest, making Chest better.You did forget one thing, the mom is not hallucinating. Isaac reads the Bible that stopped Mom and accepts he is corrupt with sin and either goes to Hell by killing himself in the chest or laying his pictures in the Chest and accepting he has to live on. The way I see it, in the game Isaac is in a sort of three part conflict, a battle for the fate of his Mind (fighting monsters and demons in his fantasy world), Body (struggling to breathe in the chest), and Soul (trying to cope with his own sin, and trying to find true salvation). The Mom fight (Not the heart) epilogue scene which shows Isaac's hallucinating Mom (She is insane from Maggy's death and Dad's leaving) stopped by God. The Chest ending is Isaac beating his death wish which is ? and then laying his memories inside of the chest.
